Are my children eligible?
Children under the age of 19 may join the Univera Community Health - Child Health Plus program
if they meet the following New York State Guidelines:

  • Must be a resident of Allegany, Cattaraugus Chautauqua, Erie or Niagara counties.
  • Must have little or no health insurance and can not be Medicaid eligible
  • Meet specific family income guidelines defined by the state to determine
    if your child will be eligible for free or low cost coverage. Coverage is
    available for the full premium if household income exceeds the guidelines.

Can I choose my child’s doctor?
Yes. Select a primary care doctor from the Univera Community Health provider directory. This provider becomes your child’s primary care doctor who will provide or arrange for all of your child’s care needs. Care is by appointment except in emergencies.

Children who enroll in Child Health Plus through Univera Community Health will have access to all Univera Community Health affiliated health centers, offices, and doctor practices located throughout throughout Allegany, Cattaraugus, Chautauqua, Erie and Niagara counties.

What does it cost?
There are no copayments for covered benefits. Depending on your family income, you may have to pay a contribution toward the premium. The contributions are $0, $9, or $15 per child based on your household size and gross family income. If your household income exceeds the state's income guidelines, you can enroll your child for the full premium of $119.72. The monthly contribution is capped at three children.

What is covered?
The coverage includes:

  • All routine visits to the doctor's office for checkups, sickness or minor injury
  • Visits to specialists
  • Prescription drugs and non-prescription drugs

  • Lab work, x-rays and other diagnostic services
  • Emergency services including emergency ambulance
  • Inpatient hospital services for medical or surgical care
  • Outpatient surgery
  • Durable medical equipment, prosthetics, orthotics
    Short-term outpatient services (chemotherapy and hemodialysis)
  • Speech and hearing services including hearing aids
  • Limited inpatient and outpatient treatment of alcoholism, substance abuse,
    and mental health
  • Diabetic supplies and equipment
  • Dental care - CHP Dental Brochure
  • Vision services including lenses and frames
  • Rehabilitation services: physical / occupational therapy

How do I enroll?
Call Univera Community Health at (716) 504-0560 or toll free at 1 (800) 494-2215 to request an application or download and print a copy.

The parent or guardian will need to complete an application and submit proof of the child's birth date (copy of the birth certificate; passport; or hospital, school or religious record), proof of residency (copy of a phone or utility bill), and proof of household's income level (copy of W-2 forms, paycheck stub of last 4 consecutive weeks, or signed income tax return - If dated after March 31 of a given year, must be accompanied by another form of documentation) If you need assistance, a marketing staff member of Univera Community Health will explain the Child Health Plus program and help complete the application.

What about in an emergency?
If you believe you have an emergency, call 9-1-1 or go to the nearest emergency room.
BUT, call your primary care doctor as soon as you can.

If you’re not sure, call your primary care doctor.

Tell the person you speak with what is happening, Your primary care doctor will:
• tell you what to do at home, or
• tell you to come to the primary care doctor’s office, or
• tell you to go to the nearest emergency room.

If you are out of the area when you have an emergency:
Go to the nearest emergency room.
Call Child Health Plus as soon as you can (within 48 hours if you can)


Preventive Health Guidelines
Univera Community Health is a strong supporter of preventive health services. We also recommend an active lifestyle, weight management and tobacco free homes. These are the recommendations for the average person in good health. Please discuss these recommendations with your physician. Depending on your health and family history, your physician may advise you to have more frequent examinations and/or additional tests.
