Q: Are there any phone numbers to help answer any questions I may have about my
health insurance?
A:
Yes, the most important number you can use to ask questions is on the back of your
member identification card.

Q: What if I move or change my phone number?
A:
It is very important to contact us if you change your address or phone number. Please
call the number on the back of your member identification card. You should also call
your case worker at the local Department of Social Services.

Q: What if I don’t have a doctor’s name listed on my member identification card or I have
the wrong doctor listed on my card?
A:
It is very important to have your doctor’s name listed on your card. Customer Service
staff are available Monday through Thursday 8 a.m. – 5 p.m. and Friday 9 a.m. – 5 p.m.
to update your doctor’s name. This is the same number that can be found on your
member identification card.

Q: If I feel sick after the doctor’s office closes, where can I go?
A:
First, call your doctor for advice. Your doctor will tell you what you should do. All doctors
can be reached 24 hours per day seven days per week. There are also urgent care or after
hours centers that may be located near you. Remember, urgent care centers are not for life
threatening emergencies.

After Hours Medical Care is available at the following Lifetime Health Medical Group locations:
  • Amherst University Health Center, 1185 Sweet Home Rd, Amherst - MAP
  • West Seneca Health Center, 120 Gardenville Parkway, West Seneca - MAP
    Hours are Monday through Friday, 5 – 8 p.m.,
    Saturdays 1 – 5 p.m. and Sundays 10 a.m. – 5:30 p.m.

Q: When do I need to call my case worker at the local Department of Social Services?
A:
You should call your Case Worker if you change your name or address. You should also call
your Case Worker if you lose your Benefit Card, become pregnant, or if your income changes.

Q: I have two cards that were given to me. How do I know when to use them?
A:
Some members might have two cards. The paper card is your member identification card.
This card is used when you go to the doctor’s office or the hospital. The plastic card is your
Benefit Card. This card is used for dental and pharmacy services. If you have questions, you
can call the number on the back for your member identification card.

Q: What if I don’t have transportation to my doctor’s appointment?
A:
Non-emergency transportation is covered in Niagara County only. If you live in any other
county, you should contact your local Department of Social Services to arrange for
transportation services.
