Univera Community Health is pleased to offer a range of programs to satisfy our members needs. Please select from the list below to see information about our member programs.


Advance Care Planning

Advance Care Planning is to help improve the quality of care at the end-of-life. The program encourages you to talk to your loved ones about your wishes at the end-of-life. You will need to complete the New York State Advance Care Directives.

To protect your wishes, make sure that you talk to your loved ones ahead of time about your medical care in the event that you can’t speak for yourself. You will need to complete advance care directive forms, including a Health Care Proxy and Living Will. This will ease the burden on families confronted with making difficult choices for loved ones who are facing life-threatening illnesses or injuries.

A Program Just for You and Your Baby

Univera Community Health knows you want to have a healthy pregnancy and a healthy baby. Our Case Management program helps you do this.

What is the program about?
The Birth and Beyond program is a free service you can use during your pregnancy if you have PlusMed, Child Health Plus, or Family Health Plus insurance through Univera Community Health.  

Your doctor has a special plan of care for you during your pregnancy.   We work with you and with your doctor to answer questions you may have about your pregnancy, your insurance coverage or any other questions.  

Getting your baby care right after he or she is born is best.   We will help you do this.

How does the program work?
The Case Management team will visit you in your home to discuss your pregnancy and answer your questions.   At our first visit we welcome you to our program with a small gift, and explain our services to you.

We can also help you get some services while you are pregnant.   You can find out about prenatal classes and review the appointments you have scheduled with your doctor.   We will also telephone you during your pregnancy to see how you are doing.

Right after your baby is born, we come for another visit. We can assist you in selecting a doctor and help you make the baby's appointment from your home. At this visit, we take a picture of your baby for you.

Also, if you go to your post partum appointment 21-56 days after the delivery, you can receive a $25 gift certificate from Univera Community Health!

We will phone you several times during your baby's first year of life to find out how your child is doing.

How do I join Univera Community Health Case Management Program?

Any pregnant Univera Community Health member who has PlusMed, Child Health Plus or Family Health Plus can join the Case Management program.

Your doctor may be the one who tells you about our program. He or she will fill out all the papers we need. Then we will call you at home, or

  • You can call us yourself as soon as you know you are pregnant, or
  • You can talk to your doctor about the program and ask him or her to call us for you, or
  • We may call you directly once we find out about your pregnancy.

Care Calls is a telephone-based program that is designed to educate and support members who have chronic illnesses, such as diabetes, asthma, depression, congestive heart failure and now smoking cessation.

members by telephone to assist with
their medical needs.

The program emphasizes on:

  • patient education
  • supportive intervention
  • self-management
  • self-monitoring skills

The Care Calls program is free to members, offers educational materials and self-monitoring tools such as asthma-peak flow meters and spacers for members with asthma, blood glucose meters for members with diabetes, an activity log for members with depression, and a monthly weight log for members with heart failure.

Highlights of the program include:

  • one-on-one instruction and coaching by telephone
  • a registered nurse who coordinates care with a member’s physician
  • flexible service hours

The program was designed and is directed by primary care and specialty physicians utilizing evidence-based clinical guidelines.

Coping with Childhood Illnesses

The mission behind CompassionNet is that no child should face unnecessary pain. And all families can hold on to the hope of healing.

When a child has a life-threatening illness, the family is often affected physically and emotionally. CompassionNet provides the support families need during their child's illness. CompassionNet coordinates services for children with serious health conditions and their families.

CompassionNet expands the extent of care for very sick children and provides assistance for their families. Doctors, nurses and other health professionals may make referrals to the program.

Unlike traditional hospice care, CompassionNet accepts children who may experience full recovery.

A CompassionNet case manager develops a plan of care for each child and his/her family. The case manager coordinates social and support services beginning at diagnosis and throughout the course of the child's illness. Those services may not be a benefit under the family's health plan.

Little Things Make All the Difference
Little things seem to make the biggest difference. One child's parents just wanted respite, for an evening out by themselves. For another family, it was having their child's blood drawn at home instead of going to the hospital. And for one mom, it was seeing her daughter, who was in constant pain, receive a massage and then smile.

The CompassionNet case management program is accepting referrals from doctors. Talk to your doctor if you would like more information.



Univera Healthcare’s Fun 2B Fit

A 12-week, innovative, school-based initiative designed to encourage students in grades two, three and four and their parents to make healthier lifestyle choices by trying new foods, increasing their level of physical activity, shopping for healthy foods and preparing meals together.

The program incorporates a taste-testing program of healthy recipes for busy families; a physical activity component where kids learn about how they can have fun being fit and a shopping field trip to a local Wegmans store.

Highlights of the program:

  • Alignment with the New York State Learning Standards
  • Interactive parent tool kits
  • Healthy incentives for children who participate, including a portion control plate
  • On line resources for teachers and parents

The goals of the program are to:

  • Know what your blood pressure and cholesterol numbers are.
  • Know what your numbers should be.
  • Know how to work with your doctor to reduce your risk factors.

What you can do…

  • Have your blood pressure taken.
  • Ask your doctor what your target blood pressure should be.
  • Ask to have a lipid (cholesterol) profile drawn.
  • Write your numbers down, on your Know Your Numbers card

You should try for at least 30-45 minutes of aerobic activity on most days. Check with your doctor before you start any exercise program.

Offers Assistance to
New Moms, Dads

Are you a new parent or about to become one?

Univera Community Health is pleased to announce its affiliation with Ready, Set, Parent! a collaborative initiative of EPIC~Every Person Influences Children and Baker Victory Services.

Depending on the member’s benefit plan, the program is either free, or may include a small copayment.

Ready, Set, Parent! is a national award-winning, comprehensive program, which starts with a visit by a hospital coordinator to new parents within 48 hours of giving birth. From there, parents are invited to attend a newborn class filled with information about parenting, health and wellness. The class emphasizes the importance of reading to children right from birth, and supplies each family with an appropriate book to get them started. 

Ready, Set, Parent! also includes an eight-week workshop for parents of infants and toddlers. Topics are designed to help any parent gain new information and guidance about raising a happy and healthy baby.
