Univera Community Health to Accept Child Health Plus Walk-ins
Appointments are not needed

According to a recent report, more than 661,000 children and adults in upstate New York – or 10.9 percent of the population - were uninsured from 2005 to 2007.

To help curb this epidemic, Univera Community Health will be accepting walk-ins for people who are interested in applying for our Child Health Plus health plan only. Child Health Plus is for uninsured children under the age of 19 who are residents of Allegany, Cattaraugus Chautauqua, and Erie counties.

Our dedicated Facilitated Enrollment team will be on hand to help individuals and families apply for Child Health Plus from 10 a.m.-4 p.m., at the Lifetime Health Medical Group’s Hamburg Health Center located at 151 Elmview Avenue on the following dates: Appointments are not needed.
December 17, 18, and 21

Univera Community Health also offers Family Health Plus, and PlusMed health plans for those who live in Allegany, Cattaraugus Chautauqua, and Erie counties. To apply for these programs, or for more information, call 504-0560 to make an appointment today.
