Am I eligible?

  • You must have Medicaid and meet additional requirements of your local department of Social Services.
  • Must be a resident of Allegany, Cattaraugus Chautauqua, Erie, and Niagara counties.

Can I choose my own doctor?
Yes!
You get to choose your own primary care doctor to coordinate your care. Your primary doctor is someone who will get to know you and your family's health history and special needs so you get the right care when you need it. And your doctor is just part of a whole team of dedicated physicians and other health care professionals who are there to help you take better care of yourself.

When you enroll in PlusMed through Univera Community Health, you will have access to all Univera Community Health affiliated health centers, offices, and physician practices located throughout the area.

How much will my doctor visits cost?
It's all free if you qualify. There are no co-pays or additional costs.

What is covered?
You can see your doctor as often as you need to for regular checkups, immunizations for your children, well-baby visits and more:

  • All routine visits to the doctor's office for checkups, sickness or minor injury
  • Visits to specialists
  • Lab work, x-rays and other diagnostic services
  • Emergency services
  • Inpatient hospital services for medical, surgical, mental health, chemical dependence
  • Outpatient surgery
  • Durable medical equipment, prosthetics, orthotics
  • Short term outpatient services: radiation / chemotherapy / hemodialysis
  • Speech and hearing services including hearing aids
  • Home health care
  • Outpatient visits for mental health and outpatient detoxification services
  • Diabetic supplies and equipment
  • Vision services including lenses and frames
  • Rehabilitation services: physical / occupational therapy
  • Family planning

Dental care, transportation, pharmacy, most outpatient chemical dependence services and other services not covered by PlusMed will still be covered by Medicaid. Use your Medicaid card to access those services if and when you need them.

How do I enroll?
Call Univera Community Health at (716) 504-0560 or toll free at 1-800-494-2215 to schedule and appointment with a Facilitated Enroller. We will help you learn everything you need to know about the program. A Facilitated Enroller will explain the PlusMed program and help you to complete the application for Medicaid. You will be required to submit proof of age/identity, proof of residency, proof of household's income, proof of citizenship/immigration status along with Social Security number verification. Our Univera Community Health facilitated enrollment process will simplify enrollment and help to determine your eligibility. Most importantly, we will help you through every step in applying for your health insurance benefits.

The final determination of eligibility for Medicaid and enrollment in PlusMed will be made by the your local department of Social Services.

What about in an emergency?
If you believe you have an emergency, call 9-1-1 or go to the nearest emergency room.
BUT, call your primary care doctor as soon as you can.

If your’re not sure, call your primary care doctor.

Tell the person you speak with what is happening, Your primary care doctor will:
• tell you what to do at home, or
• tell you to come to the primary care doctor’s office, or
• tell you to go to the nearest emergency room.

If you are out of the area when you have an emergency:
Go to the nearest emergency room.
Call PlusMed as soon as you can (within 48 hours if you can)


Preventive Health Guidelines
Univera Community Health is a strong supporter of preventive health services. We also recommend an active lifestyle, weight management and tobacco free homes. These are the recommendations for the average person in good health. Please discuss these recommendations with your physician. Depending on your health and family history, your physician may advise you to have more frequent examinations and/or additional tests.
